The following documents MUST be on file with the troop for a Girl member to participate in meetings and events.
This form must be filled out if your daughter must be given medication during a meeting or event where you are not present.
It is highly recommended that these forms are filled out yearly and kept on file at the troop for High Adventure or Swimming Events.
This form must be filled out by a Physician or other Health Care Professional Listed.
This form must be filled out by a Certified Lifeguard or Instructor.
